The Custom Form Challenge is designed to accommodate different question formats within a single challenge. It is most commonly used to capture players' information as well as used as a feedback form.
Highly recommend that you utilise the Quiz Challenge to define a "right" answer, which displays the "right" and "wrong" answer as part of the post-completion message.
